                            Please note - Livestock movement files are provided as word documents. To open and use the forms you will need to do the following :

                            1. Right mouse click once on the form you wish to open. A small menu will pop up.
                            2. Select the Save target as function to save it into a folder on your hard drive (C: drive) or your Desktop. Make a note of where you have saved it.

                            Interstate movement

                            • Movement of stock to Western Australia (word document)
                              AD1071 - This form must accompany each consignment of livestock en route into Western Australia. The entry document certifies that the livestock described meet all pre-requisites.

                              IM0018 - This declaration must be attached to the waybill.

                            Liver fluke


                            • Exemption from treating and testing - application for equines (word document)
                              IM997A - Completion before approval can be granted to allow entry.

                            • Certificate of tests and treatments (word document)
                              IM0501 - Specific treatments and tests have been completed post entry. To be completed by WA based veterinary surgeon.


                            • Declaration of origin of stock for equines, ruminants & camelids from the Liver fluke test exempt area of Australia (word document)
                              IM994 - Evidence for the exemption of described stock from testing and/or pre-entry treatment.
                            • Declaration of re-entry of stock for equines departing WA and returning from the NT within 28 days of departure (word document)
                              IM0017 - This declaration provides evidence for the described stock from inspection for weed seeds and burrs and the exemption of the transport from wash-down on re-entry into WA.
                            • Declaration of re-entry of stock for equines, ruminants and camelids departing WA and returning within 49 days of departure (word document)
                              IM995 - This declaration provides evidence for the described stock on treatment and testing for liver fluke.
                            • LF081- Liver fluke entry conditions: livestock other than pigs
